COVID-19
To avoid face to face appointments if you feel unwell, have been tested positve for COVID or have recently visited a "Place of Interest".

General agreement



Therapist


•   To be available at the agreed time
•   To start and end on time
•   To offer a quiet appropriate and undisturbed space
•   To maintain safe, professional boundaries
•   To regard all contact and information as confidential unless I have reasonable doubt concerning actual safety of yourself or others, or where required by law to disclose,

 
•   Confidentiality may be broken if you or others are, in the opinion of the therapist. Seem to be in danger or at serious risk of being harmed.
And:

1.   In the UK Under the Terrorism Act (2000), which requires that the counsellor/therapist disclose any belief or suspicion of acts of terrorism.
2.   In the UK Under the Drug Trafficking Act (1986), which requires the counsellor/therapist to disclose to the police information of any individual making money through drug trafficking.
3.   In the UK Under the Road Traffic Act (2000), which requires citizens to provide information to the police that might identify a driver in a traffic offence. In addition, if a counsellor/therapist becomes aware that a client may be driving whilst unsafe (e.g. through epilepsy, medical condition, drug or alcohol abuse) the law requires the counsellor/therapist to pass this information to the DVLA.

•   To work within the PBANZ, NZAP and UKCP code of ethics
•   To review therapeutic work and relationship regularly as part of professional clinical supervision, There are times where aspects of our sessions will be taken to supervision to monitor my practice; at no time will your name or any identifiable information be mentioned and my supervisor is also committed to our contracted confidentiality.
•   Records of sessions: I keep notes relating to our sessions, this contract and your contact details. These are retained in password protected digital form on a computer. By signing this contract you agree to me doing this.
•   In the unlikely event of the me cancelling, an alternative appointment will be offered as soon as possible
•   Will not openly acknowledge you if meeting by chance, unless you acknowledge first.

Client responsibilities

•   To attend punctually
•   To give a minimum of 48 hours notice when cancelling/changing an appointment (or the full fee becomes payable)
•   To pay the agreed fee (UK:£60 per 50 minute session, NZ$130) after each session unless otherwise agreed (concessionary fees must be agreed in advance)
•   Communicating with the therapist outside agreed counselling sessions to be limited to making, changing or cancelling an appointment unless by prior arrangement. Contact may be by phone, text or email.
•   To be respectful to Eric Medcalf and his property
•   To agree to give permission to contact GP if the therapist has serious concerns about risk to self (client) or others; or when collaboration is necessary (this will be discussed)
•   To discuss with the therapist when you feel you are ready to end therapy
•   To let the therapist know if you are in or are considering entering another therapeutic relationship.
